One of the most remarkable and heartbreaking chapters in medical history is the story of Dr. Ignaz Semmelweis. A Hungarian physician working in Austria discovered a secret that saved thousands of lives.

Vienna General Hospital’s Deadly Mystery

Ignaz Semmelweis was appointed assistant doctor at the Vienna General Hospital’s First Obstetric Clinic in 1846. Women pregnant at this prestigious institution faced a terrifying reality behind its grand doors.

The mysterious illness known as puerperal fever, or childbed fever, was sweeping through maternity wards across Europe at the time. Newly-born women often developed soaring fevers, chills, and severe abdominal pain after giving birth. Many of them died within a short period of time.

Due to the hospital’s separate operation of two maternity clinics, the situation in Vienna was particularly bizarre.

  • First Clinic: Staffed by male general practitioners and medical students.
  • Second Clinic: A woman-only clinic staffed only by female midwives and midwife trainees.

There was a grim statistic Vienna women were acutely aware of. It was routinely three to four times more likely that patients would die in the First Clinic than in the Second Clinic.

Expectant mothers practically begged for their admission to the First Clinic on their knees because they were so terrified. Some women preferred to deliver in the streets instead of giving birth in hospitals. The prevalence of childbed fever is almost nonexistent among women who give birth on the street before reaching the hospital.

In spite of this discrepancy, Semmelweis was haunted by it. Under the care of highly trained doctors, mothers died at shocking rates, while under the care of midwives, they usually survived?

An Era Before Germs: Searching for Answers

A look at what doctors believed in the mid-nineteenth century can help explain why Semmelweis faced such a difficult battle. Germ theory had not yet been established at this time.

The existence of bacteria and viruses was unknown to physicians. Medicine relied instead on ancient theories to explain diseases.

The most common explanations for childbed fever

  1. Miasma Theory: The belief that diseases were caused by poisonous vapors or bad air circulating in the atmosphere.
  2. Humoral Imbalance: An imbalance of blood, bile, and phlegm caused illness.
  3. Doctors suspected that women were simply too delicate and that anxiety during birth caused fever in some women.
  4. Physical Positioning: Others speculated that the physical position women assumed during labor in the First Clinic was to blame.

In order to test each hypothesis, Semmelweis conducted a systematic experiment. Mothers’ birth positions were changed. By changing the way religious ministers walked through the wards, he avoided frightening patients.

Moreover, he tried to improve ventilation in order to get rid of the bad air. There was no difference in any of these changes. There were more and more deaths, and Semmelweis grew more desperate.

What Revealed the Truth was a Tragic Epiphany

In 1847, a deeply tragic event led to the breakthrough. Kolletschka accidentally cut his finger with a scalpel while performing an autopsy on a woman who had died of childbed fever, Semmelweis’ close friend and colleague.

Kolletschka began exhibiting the same symptoms as dying mothers within days. Before passing away, he suffered from a high fever, severe inflammation, and organ failure.

As Semmelweis reviewed the autopsy results of his friend, he noticed something startling. Kolletschka’s body had the same types of tissue damage as those found in childbed fever victims’ bodies.

The light suddenly dawned on Semmelweis. Kolletschka did not die from atmospheric vapors or an imbalance of humors. Through the scalpel cut, poison entered his bloodstream from the dead body.

As Semmelweis realized, the two clinics were fundamentally different.

In the First Clinic, autopsies were performed on deceased patients every morning by medical students and doctors. They walked directly into the delivery rooms to examine laboring mothers without washing their hands or changing their blood-stained coats.

Midwives in the Second Clinic, however, did not perform autopsies. Their hands were clean when they entered the delivery room.

It was as if the doctors were carrying invisible death directly into the bodies of young mothers from the morgue.

Cleanliness Protocol: A First in Medical History

According to Semmelweis, cadaveric particles were transferred into living patients from autopsies and caused the fatal illness.

Although doctors’ hands smelled like the morgue even after washing, he believed soap and water were not enough to remove these harmful particles.

In April 1847, before the handwashing rule was enforced, the mortality rate in the First Clinic stood at nearly eighteen percent. By July, after the introduction of the chlorine wash, the mortality rate plummeted to less than two percent.

In some subsequent months, the death rate in the First Clinic dropped to zero. For the first time in memory, the First Clinic was safer than the Second Clinic.

Semmelweis had proved beyond any doubt that clean hands saved lives.

Why the Medical World Rejected Handwashing

If a doctor today discovered a simple step that instantly reduced death rates to zero, they would be celebrated worldwide. Yet Semmelweis was met with hostility, insult, and total rejection.

1. The Offense to Medical Pride

To accept Semmelweis’s theory meant accepting a horrifying truth. Doctors had to admit that they were the ones responsible for killing their own patients.

Nineteenth-century physicians were gentlemen of high social standing. The suggestion that their hands were dirty or carried lethal contagion was viewed as an insult to their character and dignity.

2. The Lack of a Scientific Explanation

Semmelweis could show that handwashing worked, but he could not explain scientifically why it worked.

Because germ theory had not yet been proven by scientists like Louis Pasteur or Robert Koch, Semmelweis could not point to microscopic bacteria under a lens. To established physicians, the idea of invisible particles causing disease sounded like superstitious nonsense.

3. Political and Cultural Tensions

Semmelweis was a Hungarian living in Vienna during a period of intense political unrest. Hungarian nationalist movements were challenging Austrian rule, and Semmelweis’s political sympathies created friction with his superior, Dr. Johann Klein.

Klein actively disliked Semmelweis and viewed the handwashing protocol as an annoying critique of his management.

A Tragic Descent into Isolation and Loss

In 1849, Semmelweis was dismissed from his post at Vienna General Hospital. His supervisor refused to renew his contract and removed the handwashing basins from the clinic.

Predictably, as soon as handwashing stopped, the death rate among mothers soared right back to its previous catastrophic levels.

Frustrated and heartbroken, Semmelweis returned to his hometown of Pest in Hungary. He took charge of the maternity ward at St. Rochus Hospital and immediately enforced his handwashing protocol. Once again, mortality rates dropped to near zero.

Yet despite his repeated practical successes, the broader medical community refused to listen. Major medical journals published reviews mocking his work. Leading authorities in medical centers like Berlin, London, and Paris openly rejected his papers.

Semmelweis grew increasingly bitter and obsessed. He began writing open, fiery letters to prominent doctors across Europe, calling them murderers and irresponsible butchers for refusing to wash their hands.

His mental health began to deteriorate under the crushing weight of rejection and the knowledge that thousands of mothers were dying unnecessarily every day.

By 1865, his behavior had become severely erratic. He spoke of little else besides childbed fever, suffered mood swings, and began drinking heavily.

In July 1865, his wife and colleagues lured him to a mental asylum in Vienna under the pretense of visiting a new medical institute. Realizing he was being committed against his will, Semmelweis tried to escape, but guards severely beat him and placed him in a straitjacket.

Just two weeks later, on August 13, 1865, Ignaz Semmelweis died at the age of forty-seven.

In a cruel twist of fate, the cause of his death was sepsis, a severe blood infection caused by a wound on his hand, likely sustained during his struggle with the asylum guards. He died from the very same septic condition he had spent his entire adult life trying to prevent.

Vindicated by the Passage of Time

Only a few years after Semmelweis died in obscurity, the scientific world began to catch up to the truth he had discovered decades earlier.
